Welcome to the fifth edition of Specialty Contact Lens Case Reports. Reaching this 5-year milestone offers a wonderful moment to reflect on how far this subspecialty has come and how rapidly it continues to transform. What we began as a dedicated effort to capture unique clinical encounters has grown into a vital resource for eyecare
professionals in their complex contact lens management.
This year, we are proud to present 14 real-world case studies contributed by experienced practitioners from the United States and across the globe. Each report goes beyond standard clinical summaries, providing a look into lens design options and problem-solving strategies. Importantly, they highlight the impact that customized contact lenses have on restoring vision, comfort, and daily quality of life for our patients.
This collection reflects the expanding sophistication of the modern specialty contact lens practice. Explored here is using scleral lenses for severe ocular surface and corneal compromise, as well as complex ectasias, postsurgical irregularities, and severe dry eye disease. As an example, the use of advanced landing zone
customization and impression-based fitting strategies is discussed.
Cases using custom soft and hybrid designs for irregular astigmatism are described, demonstrating how advanced lathed geometries and tailored center-thickness profiles provide compelling alternatives when rigid modalities are less tolerable.
Specialized multifocal and presbyopic solutions overcoming complex optical higher-order aberrations to deliver functional binocular vision across all working distances are also presented.
These reports serve as a powerful reminder that specialty lenses are no longer reserved as a “last resort” when traditional options fail. Supported by advancements in corneo-scleral topography, profilometry, and precision manufacturing, specialty fitting has firmly established itself as a proactive, primary therapeutic approach.
